Blood pressure medications don't all work the same way — some target a hormone system, others relax blood vessels directly, and others reduce overall blood volume. Understanding the categories helps explain why more than one is often needed.
Blood pressure readings and the medications used to manage them both make considerably more sense once the specific numbers and mechanisms behind them are understood, rather than treating "high blood pressure" as a single undifferentiated category.
Understanding the Numbers: What the Categories Actually Mean
Current guidelines generally divide blood pressure into categories — normal, elevated, stage 1 hypertension, stage 2 hypertension, and hypertensive crisis — with each category reflecting both a different level of cardiovascular risk and a different general urgency for treatment. Elevated readings that don't yet reach stage 1 are generally addressed with lifestyle measures first, while stage 2 readings more often prompt medication in addition to lifestyle changes, and a hypertensive crisis reading represents a situation requiring urgent medical attention rather than a routine follow-up.
Why Two Numbers Matter, Not Just One
The systolic number (the higher of the two) reflects pressure during the heart's contraction, while the diastolic number (the lower one) reflects pressure during the brief relaxation period between heartbeats. An elevated reading in either number carries meaningful cardiovascular risk on its own — it isn't the case that only a high systolic number matters, or that a normal systolic number with an elevated diastolic number can be safely ignored.
ACE Inhibitors and ARBs: Targeting a Specific Hormone System
The renin-angiotensin system is a hormone cascade in the body that constricts blood vessels and promotes sodium retention, both of which raise blood pressure. ACE inhibitors and angiotensin receptor blockers (ARBs) work by blocking this system at different points along the same pathway, lowering blood pressure through this specific hormonal mechanism rather than a generic, non-specific effect.
Calcium Channel Blockers: A Direct Approach to Vessel Relaxation
This class works through a different pathway entirely: by blocking calcium from entering the smooth muscle cells lining blood vessels, they directly relax those vessels, reducing resistance to blood flow. This is a mechanically distinct approach from the hormone-system-based classes above, targeting the blood vessel itself rather than a signaling pathway upstream of it.
Diuretics: Reducing Volume Rather Than Resistance
Diuretics work through yet another distinct mechanism, increasing sodium and water excretion through the kidneys and thereby reducing overall blood volume. Since blood pressure is directly related to the volume of blood the heart has to move, reducing that volume reduces the pressure needed to move it — a volume-based approach rather than a vessel-relaxation approach.
