One might feel steady throughout the day; the other might swing between energy crashes and jittery spikes repeatedly. The difference isn't the average. It's the size and speed of the swings around it, something standard testing doesn't capture at all.
What "Blood Sugar Fluctuation" Actually Means
Glycemic variability — how much and how quickly blood sugar rises and falls — is increasingly recognized as its own factor in how someone feels and, over time, in cardiovascular risk, somewhat independent of the average glucose level itself. This matters for a wide range of people, not just those with diagnosed diabetes: continuous glucose monitoring in people without diabetes has revealed that meaningful swings are far more common than previously assumed, often triggered by ordinary meals that wouldn't be flagged as a concern on any standard test.
Why "Normal" Average Blood Sugar Doesn't Rule This Out
Because HbA1c reflects an average, it can look identical for someone whose glucose stays fairly flat all day and someone whose glucose spikes sharply after meals and then crashes just as sharply — the swings essentially cancel out mathematically even though the lived experience is very different. This is part of why someone can be told their labs are "fine" while still regularly experiencing symptoms tied to glucose swings.
Common Symptoms of the Swings Themselves
- Shakiness, irritability, or difficulty concentrating, typically 2–4 hours after a carbohydrate-heavy meal, as glucose drops sharply following an exaggerated insulin response.
- A sudden urge to eat something sugary, which is the body's response to a rapid glucose drop rather than genuine hunger.
- Brain fog or a noticeable energy crash in the early afternoon, often following a high-carbohydrate lunch.
- Heart palpitations or a sense of internal shakiness, driven by the adrenaline release that accompanies a rapid glucose drop.
- Excessive sweating unrelated to temperature or exertion, another common accompaniment to a glucose crash.
The Specific Pattern Behind Many of These Symptoms: Reactive Hypoglycemia
When a large, rapidly digested carbohydrate load hits the bloodstream, the body can release more insulin than the situation actually requires, overshooting and driving glucose down below a comfortable range a few hours later. This pattern — reactive hypoglycemia — is common even in people with completely normal glucose regulation otherwise, and it's frequently mistaken for general tiredness or low blood sugar "just happening" rather than recognized as a predictable response to a specific type of meal.
What Actually Causes the Swings
- Carbohydrates eaten without protein, fat, or fiber are digested and absorbed quickly, producing a sharper rise and a correspondingly sharper fall.
- Skipping meals and then eating a large one later tends to produce a bigger swing than smaller, more evenly spaced meals.
- Alcohol, particularly on an empty stomach, can cause delayed glucose drops several hours after drinking.
- Poor or insufficient sleep measurably increases next-day glucose variability, even without any change in diet.
- In people using glucose-lowering medication or insulin, mismatches between dosing and meal timing or composition are a common and often correctable cause of swings.
