Peripheral neuropathy — nerve damage outside the brain and spinal cord — has dozens of possible causes, but its telltale symptoms and a systematic evaluation usually narrow things down considerably.

Peripheral neuropathy isn't a single disease but a description of damage to the peripheral nervous system — the vast network of nerves that carries signals between the brain, spinal cord, and the rest of the body. Because that network is so extensive, damage can present in many different patterns depending on which nerves are affected, how many, and whether the damage involves sensory nerves, motor nerves, autonomic nerves, or some combination — which is exactly why the same broad label covers presentations that can look quite different from one person to the next.

The Telltale Symptom Pattern

The most classically recognized pattern is a symmetric, "stocking-glove" distribution — numbness, tingling, or burning pain that starts in the toes and fingers and gradually moves upward, affecting the longest nerves first simply because they're more vulnerable to whatever process is causing the damage. Sensory symptoms include numbness, tingling ("pins and needles"), burning or shooting pain, and a heightened, sometimes painful sensitivity to light touch (allodynia). When motor nerves are involved, muscle weakness, cramping, and, in more advanced cases, muscle wasting can occur. Autonomic nerve involvement — less commonly recognized as neuropathy by patients — can cause symptoms like abnormal sweating, dizziness on standing due to blood pressure regulation issues, or digestive changes.

Diabetes: The Single Most Common Cause

Diabetic peripheral neuropathy is by far the most common cause of peripheral neuropathy in regions with high rates of diabetes, developing in a significant proportion of people with long-standing or poorly controlled diabetes. Chronically elevated blood glucose damages small blood vessels that supply the nerves themselves, gradually starving them of oxygen and nutrients, and directly damages nerve fibers through several biochemical pathways involving oxidative stress and abnormal protein accumulation. The risk and severity of diabetic neuropathy correlate fairly closely with the duration of diabetes and the degree of blood glucose control over time, which is why glucose management remains the single most effective way to slow its progression once diagnosed, even though existing nerve damage often doesn't fully reverse.
