Insulin resistance often develops years before a standard blood sugar test would ever flag it. Here are the earlier warning signs, what's actually happening in the body, and what genuinely improves insulin sensitivity.
By the time a routine blood test shows elevated fasting glucose, insulin resistance has often already been present for years. That's because the body compensates remarkably well at first — which means the earliest, most useful window for intervention is usually well before standard testing would ever catch it.
What's Actually Happening in the Body
Insulin's job is to signal cells — particularly in muscle, fat, and liver tissue — to take up glucose from the bloodstream. In insulin resistance, those cells become progressively less responsive to that signal. The pancreas compensates by producing more insulin to force the same effect, a state called hyperinsulinemia. For a long time, this compensation works well enough that blood glucose stays in a normal range, masking the underlying problem entirely.
The trouble is that this compensation isn't free. Chronically elevated insulin levels have effects of their own — including promoting fat storage (particularly visceral fat around the abdomen) and contributing to blood vessel changes associated with cardiovascular risk, well before a diabetes diagnosis would ever appear on paper.
Why Standard Tests Often Miss It Early
A fasting glucose test only becomes abnormal once the pancreas can no longer fully compensate — which is a relatively late stage in the process. Two other measures tend to flag the problem earlier: fasting insulin levels and the HOMA-IR calculation (which estimates insulin resistance from fasting glucose and insulin together). Neither is part of routine bloodwork by default, which is why many people are told their labs are "normal" for years despite insulin resistance already being active.
Early Signs Worth Paying Attention To
Several signs tend to show up well before a diagnosis, though they're easy to dismiss individually:
- Energy crashes 1–2 hours after carbohydrate-heavy meals — a pattern that reflects exaggerated insulin swings rather than simple tiredness.
- Increased hunger shortly after eating, even after a full meal, as cells struggle to take up the glucose that's still circulating.
- Darkened, velvety patches of skin (acanthosis nigricans), typically at the neck, armpits, or skin folds — a direct visible marker of elevated insulin.
- Skin tags, particularly multiple small ones appearing in the same areas, are associated with insulin resistance more often than commonly recognized.
- Disproportionate abdominal weight gain that resists typical diet and exercise efforts, often linked to how insulin promotes visceral fat storage specifically.
None of these alone confirms insulin resistance, but a cluster of several is a reasonable reason to ask a healthcare provider about fasting insulin testing specifically, rather than glucose alone.
What Drives It
Visceral fat — the fat stored around abdominal organs rather than just under the skin — is metabolically active tissue that releases inflammatory signals directly interfering with insulin's ability to work efficiently. This is part of why waist circumference is often a more informative marker than body weight alone. Chronic poor sleep, prolonged sedentary behavior, and ongoing low-grade inflammation compound the effect, alongside a genetic component that makes some people more susceptible at a given body weight than others.
